Procedure
for Registering Uniqiue Code for Trauma Centres
identified under 11th Plan for G-Q, N-S, E-W highways
For acquiring Unique Agency Code (UAC) the following steps
are necessary:-
1.
Go to the website: http://www.cga.nic.in/
2.
Now go to the link on the
right side captioned “Central Plan Scheme Monitoring System”. (CPSMS)
3.
On clicking the CPSMS
link on the Right side go the link “Agency Register”.
4.
On clicking “Agency
Register” we would need to fill in the details.
5.
The Type of
Registration is “State Govt. (Institutions)”.
6.
Agency Name: Please
fill in the name of Hospital which has been identified for setting up Trauma
Care Centre.
7.
Act/Registration
No.: Not mandatory.
8.
Date of Registration:
Date when you are registering with the website for UAC.
9.
Registration
Authority: The Authority who has been authorised to Register. For e.g.
MS/Principal/Director of the concerned Institute.
10.
State of Registration: Indicate name of your
state.
11.
TAN No.: The No allotted to the Institute for
Tax purposes.
12.
Unique Agency Code: You will give an alpha
numeric value as indicated in the site itself
e.g.
Scheme/Bank Details.
13.
Scheme: Please select “Assistance to State for
Capacity Building in Trauma Care (0409)”.
14.
Bank: Please give the name of Bank where the
Institute has opened their Account for this trauma care scheme along with A/C
No. and Agency Name as per Bank records.
15.
Now please submit
after ensuring all details.
